Coaching Agreement for
The Group Client Program

Session Day/Time:
|
We will typically meet by telephone for 45 minutes per session. We will agree upon calls for a specific time and day of the week. A one-time \"intake\" session will occur at the beginning of the coaching program, to assess coaching readiness and set initial goals.

Scope of Services:
|
We will work to define your goals, selecting the most important goal you wish to work on. We will also identify your desired outcome, explore opportunities and challenges, and ultimately establish an action plan to achieve your goal. Your results depend directly on your own efforts. Specific details of your goals will be outlined during the initial intake session.

Changes / Cancellations:
|
If you need to reschedule, please let me know at least 48 hours in advance by phone, email or fax. You will be charged for cancellations or missed calls.

Session Fees and Payments Terms:

Accepted payment methods are credit card or check. If you are writing a check, please make checks payable to Empowerment Unlimited Coaching, LLC. Payment must be made at the beginning of each month. There will be a $20 fee for returned checks. Invoices can be sent upon request but you are responsibly for timely payment.

Call Procedure:
|
You will call me at the prearranged number on the day and time scheduled. You will be responsible for all telephone related charges. The allotted time will begin when the call is placed. I encourage brief phone calls or e-mails in between coaching calls if you have important concerns or ideas you want to share.

Contract Period:
|
This agreement may be extended on a month-to-month basis following the initial agreement period with the mutual agreement of both parties. Either of us can cancel this agreement at any time. Cancellations must be in writing, fax or email. Payment for the current month will be considered payment in full, unless there is an unpaid balance. Regardless of which party cancels the agreement, you will remain responsible for payment of all fees and costs incurred prior to notice of cancellation.

Confidentiality

I recognize that certain information of a confidential nature may be relayed during our calls or other informal communication. I will not, at any time, either directly or indirectly use this information for my benefit, nor disclose this information to anyone else without your specific approval, except as reasonably required for me to perform the required coaching services or as required by my professional ethical standards related to disclosure of illegal activities or as compelled by court order.

You have been made aware that the
COACHING RELATIONSHIP IS IN NO WAY TO BE CONSIDERED OR CONSTRUED AS PSYCHOLOGICAL COUNSELING OR ANY TYPE OF THERAPY. You understand that Empowerment Unlimited Coaching, LLC (EUC) makes no warranties or guarantees, express or implied, regarding the services provided under this Agreement. All implied warranties are expressly disclaimed and excluded. You agree that EUC's or its officers', employees' or agents' liability to you for any breach, error, or omission or negligent act arising out of the performance of its services under this Agreement shall be restricted to the amount of money paid by you for EUC's services; such remedy is your sole and exclusive remedy.
You also agree to indemnify, defend, hold harmless and release EUC and its officers, employees and agents from any and all
claims that result from or are related to the performance of this Agreement except to the extent they are caused by EUC's willful misconduct.
Arizona law governs this Agreement and the Maricopa County Superior Court has jurisdiction over any litigation arising under this Agreement. This Agreement constitutes the entire understanding of the parties, and there are no representations, warranties, or undertakings made other than those established in this Agreement. This Agreement may be amended, modified, or terminated in writing and signed by all parties to this Agreement.
